Job summary

Qualifications

• Must possess a current, valid RN license in state of practice.
• BLS certification is required.

Responsibilities

• Assess patient physical, psychological, social, spiritual, developmental, and discharge planning needs.
• Formulate and implement a plan of care, including discharge planning.
• Coordinate and supervise care, delegating tasks and communicating with the care team.
