Qualifications

• Bachelors in Physical Therapy from an accredited university (APTA approved) required.
• Masters or Doctorate in Physical Therapy preferred.
• Arizona Physical Therapy license in good standing required.
• Basic Life Support (BLS) certification required.

Responsibilities

• Evaluate and treat patients to restore function and mobility.
• Develop and update individualized plans of care with patient/family.
• Collaborate with physicians, care team and supervise therapy assistants as needed.
