Qualifications

• Driver's license only if Home Care department.
• New graduates may be mentored by a senior PT.

Responsibilities

• Direct patient care and implement plan of care.
• Evaluate patients and develop treatment plans with reassessment.
• Document evaluations, plans, progress, and discharge planning.
• Coordinate with physicians, nurses, case managers, and staff.
• Mentor PTAs and rehab techs; participate in team meetings.
