Job summary

Responsibilities

• Evaluate patient physical abilities and plan care.
• Perform direct patient care 90–100% of the time.
• Develop and implement plans of care with reassessment.
• Supervise PTAs and rehabilitation techs.
• Coordinate discharge planning with the multidisciplinary team.
• Document evaluation, plan of care, progress, and discharge details.
• Contribute to department quality improvement and meetings.
