Responsibilities

• Direct patient care 90-100% with evaluation and treatment.
• Develop and implement plan of care and reassess as needed.
• Coordinate discharge planning with multidisciplinary team.
• Document evaluations, plans, progress, and discharge needs.
• Supervise PTAs and rehab techs; participate in quality improvement.
