ThePalliative CareSocial Worker, a critical member of the Care Transitions team, is responsible to assess patient's social and behavioral needs, provide focused support to patients to address health inequities and aims to address the social, emotional, and environmental factors impacting their well-being. The Social Worker collaborates closely with thePalliative Care team includingphysicians, nurses, chaplains, community health workers, inpatient Care Transitions team, and theoutpatient Palliative Care team to enhance patient outcomes, reduce length of stay, reduce readmissions, and improve the overall patient experience.

MINIMUM QUALIFICATION(S):

• Masters of Social Work degree from a Council of Social Work Education school of social work

PREFERRED QUALIFICATION(S):

• Minimum of two (2) years of acute care case management/social work experience
• Knowledge of PC based computer software (Cerner, Navihealth/Curaspan)
• Current social work licensure in Pennsylvania
• Certification in case management from a nationally recognized organization
