Job summary

Palmdale Regional Medical Center, part of Southwest Healthcare's network, is seeking a Licensed Vocational Nurse to join our Case Manager team in the inpatient setting at Palmdale, CA.

Under the Director of Case Management, you will coordinate patient care with an interdisciplinary team and contribute to utilization review and discharge planning to optimize outcomes within the constraints of resources available to patients and families.

Qualifications

• Minimum five years of acute care clinical experience.
• Minimum of three years in Case Management/Utilization Review (preferred).
• Graduation from LVN program with California license.
• Current BLS certification.

Responsibilities

• Coordinate the delivery of health care services for the inpatient population.
• Serve as an interdisciplinary team member under the Director of Case Management.
• Expert in utilization review and discharge planning to support quality outcomes within available resources.
