Job summary

MJHS is seeking an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships and assess patients referred for hospice enrollment. The role provides General Inpatient care with specialized nursing skills and leads the interdisciplinary team to develop individualized care plans for patients.

The coordinator educates referrers, patients, and families about hospice, coordinates with healthcare providers, and ensures accurate documentation in accordance with regulations

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Minimum of two years’ experience in Medical-Surgical or Community Health. Hospice or Oncology nursing experience preferred.
• New York State RN licensure, CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.

Responsibilities

• Develops and strengthens business relationships with hospital partners.
• Assess patients referred for hospice enrollment and provide GIP level of care.
• Evaluate referred patients and facilitate admission to hospice per laws and policies.
• Coordinate with inpatient providers and staff; develop individualized care plans with the IDT.
