Job summary

MJHS is seeking an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships, assess patients for hospice enrollment, and provide General Inpatient care with specialized clinical skills.

You will lead comprehensive assessments, develop care plans with the interdisciplinary team, educate referrers and families, coordinate with hospital and home hospice teams, and ensure accurate documentation in accordance with regulations.

Qualifications

• RN licensure in NYS and CPR-BLS.
• (2+) years in Medical-Surgical or Community Health; hospice/oncology experience preferred.
• Flexible hours including weekends.
• Ability to coordinate resources in an interdisciplinary team.

Responsibilities

• Develops comprehensive assessments and collaborates with inpatient providers and staff.
• Coordinates transition plans for patients no longer qualifying for the inpatient level of care.
• Educates referrers, patients, and families about hospice services.
• Maintains accurate and current documentation reflecting hospice nursing assessments and interventions.
• Serves as RN member of the Inpatient Hospice Interdisciplinary Team (IDT) for GIP patients.
