Job summary

MJHS is seeking a skilled Hospice Inpatient Clinical Coordinator to cultivate hospital partnerships and assess patients for hospice enrollment. You will deliver GIP care and lead an interdisciplinary team to develop individualized care plans with compassion and adherence to policy.

The role requires strong communication, organizational, and problem-solving abilities, with flexibility including weekends. Experience in oncology or hospice settings is preferred, and NY licensure is required.

Qualifications

• RN licensure in New York State; CPR-BLS.
• BSN preferred; accredited School of Nursing.
• Minimum two years’ experience in Medical-Surgical or Community Health; Hospice/Oncology nursing preferred.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours, including weekend hours.
• Ability to coordinate the resources and personnel involved in a comprehensive interdisciplinary team environment.
• Ability to work non‑judgmentally with patient/family members of any culture, religion, socio‑economic background or lifestyle.

Responsibilities

• Develops strong business continuity relationships to support the agency’s business development goals.
• Responds to all requests for information and referrals from hospital personnel and obtains the Election of Benefits for hospice enrollment of eligible patients.
• Gathers clinical information used in eligibility determinations, utilizing Hospice Conditions of Participation, LCDs, and state/federal regulations.
• Coordinates effective symptom management for patients awaiting hospital discharge to home hospice with referring physicians and hospital case management/SW staff.
• Serves as the RN member of the Inpatient Hospice Interdisciplinary Team (IDT), providing daily coordination of hospice care for GIP patients.
• Develops the hospice nursing plan of care and makes recommendations for services based on patient/family needs.
• Participates in Interdisciplinary Team Meetings (IDT) weekly and GIP Huddle daily to communicate patient status.
• Coordinates the transition plan for patients who no longer qualify for the inpatient level of care.
• Conducts oversight of clinical care provided by hospital staff and educates staff on hospice care principles.
• Maintains accurate and current documentation reflecting hospice nursing assessments and interventions.
