Job summary

MJHS Hospice and Palliative Care seeks an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships, assess referrals for enrollment, and deliver General Inpatient level care with compassion and precision.

You will lead an interdisciplinary team, develop individualized care plans, educate providers and families about hospice services, and ensure thorough documentation while navigating applicable regulations and agency policies.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Two years’ experience in Medical-Surgical or Community Health; hospice or oncology nursing preferred.
• New York State RN licensure; CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours, including weekend hours.
• Ability to coordinate resources and personnel in an interdisciplinary team environment.
• Ability to work non-judgmentally with patients/families of any culture.
• Excellent communication, listening, and organizational skills.
• Ability to speak in public.
• Ability to demonstrate effective critical thinking skills.
• Ability to solve problems independently.

Responsibilities

• Develops and strengthens hospital partner relationships to support business development goals.
• Responds to information requests and referrals for hospice enrollment.
• Gathers clinical information for eligibility determinations per OP, LCDs, and regulations.
• Coordinates symptom management for patients awaiting discharge to home hospice with referring physicians and hospital staff.
• Serves as RN member of the IDT, coordinating hospice care for GIP patients.
• Develops the nursing plan of care and recommends services based on patient/family needs.
• Participates in IDT meetings and daily GIP huddles to communicate patient status.
• Coordinates transition plans for patients no longer qualifying for inpatient care.
• Educates hospital staff about hospice care and maintains current documentation.
