Job summary

MJHS is seeking an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships, assess referrals, and coordinate General Inpatient hospice care with compassion. The role includes comprehensive patient assessments with the IDT, education for referrers and families, and meticulous documentation to ensure regulatory compliance and quality care.

Candidates should be a BSN-preferred RN with 2+ years in Medical-Surgical or Community Health, NY RN license, CPR/BLS, and

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• A minimum of two years’ experience in Medical-Surgical or Community Health; hospice/oncology preferred.
• New York State RN licensure and CPR-BLS certification required.
• Valid NYS driver's license preferred; familiarity with Microsoft Office applications.
• Ability to work flexible hours, including weekends, and coordinate interdisciplinary resources.
• Ability to work non-judgmentally with patients and families from diverse backgrounds.

Responsibilities

• Develops strong business continuity relationships to support the agency’s goals with referral partners.
• Responds to requests for information and referrals from hospital personnel and coordinates hospice enrollment.
• Gathers clinical information for eligibility determinations in line with Hospice guidelines and LCDs.
• Coordinates symptom management for patients awaiting discharge to home hospice with referrers and case managers.
• Acts as RN member of the Inpatient Hospice IDT, coordinating care for General Inpatient patients.
• Maintains the hospice nursing plan of care and recommends services based on patient/family needs.
