Job summary

Qualifications

Responsibilities

• Provide and coordinate clinically competent care for an assigned group of patients.
• Document patient data and adjust treatment plans as indicated.
• Educate patients and families about learning needs.
• Delegate tasks to PCTs/LPNs as appropriate.
• Coordinate communication with dialysis providers to ensure continuity of care.
• Participate in quality improvement activities.
