Cleveland Clinic Fairview Hospital is hiring a part-time Clinical Pharmacist for inpatient services. The role involves rotating between central pharmacy operations and unit-based care, ensuring safe and effective medication management across the hospital.

The ideal candidate will hold a PharmD or BS Pharmacy, maintain state licensure as an RPh, and have current BLS (and ACLS/PALS where applicable). Weekend coverage and shift rotations are required.

Formación

• PharmD or BS Pharmacy from an accredited college of pharmacy.
• Current state licensure as an RPh or eligible for reciprocation within 90 days of hire.
• BLS certification and immunization administration training within 90 days.
• ACLS and/or PALS certification; required within one year for relevant roles.

Responsabilidades

• Verify and clarify medication orders and dispense accurately.
• Oversee IV preparation and sterile product processes.
• Evaluate drug therapy and optimize dosing; monitor interactions and safety.
• Provide pharmacokinetic consultations as applicable.
• Cross-train in areas like critical care and ED; participate in rounds when scheduled.
