Job summary

Cleveland Clinic Fairview Hospital seeks an experienced Clinical Pharmacist - Inpatient on a 7-on/7-off evening schedule (1:00 pm–11:30 pm). The role provides comprehensive medication therapy management and supports interdisciplinary rounds to optimize patient care.

You will verify orders, optimize dosing, monitor interactions, and educate staff and patients. A PharmD or BS in Pharmacy with active RPh license and BLS/ACLS/PALS credentials is required, with immunization certification within 90

Qualifications

• PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
• Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days of hire.
• Current Basic Life Support (BLS) certification; ACLS and/or PALS within 12 months and immunization certification within 90 days.

Responsibilities

• Verify medication orders and dispense accurately according to state and federal regulations.
• Provide clinical pharmacotherapy evaluation and dosing optimization.
• Participate in interdisciplinary rounds and educate patients.
• Support sterile product preparation and ensure medication safety and regulatory compliance.
• Document services in a timely and complete manner.
