Job summary

Cleveland Clinic Fairview Hospital, part of the Cleveland Clinic Health System, is seeking an inpatient Clinical Pharmacist for 7 on/7 off evenings. You will perform medication order verification, dose optimization, and pharmacokinetic consultations while supporting sterile product preparation and interdisciplinary rounds.

Required are PharmD or BS in Pharmacy, state licensure as an RPh, BLS (with immunization certification) and ACLS/PALS eligibility or certification; sign-on and relocation

Qualifications

• PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
• Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days of hire.
• Current Basic Life Support (BLS) certification from AHA or Red Cross and immunization certification or within 90 days.
• Current ACLS and/or PALS certification from AHA or Red Cross; required within one year for cardiopulmonary arrest response.

Responsibilities

• Provide clinical and operational pharmacy services for hospitalized patients.
• Verify medication orders and dosing; optimize drug therapy and monitor interactions.
• Conduct pharmacokinetic consultations and support sterile product preparation.
• Participate in interdisciplinary rounds and ensure regulatory compliance; assist with medication safety.
