You will collaborate with the healthcare team to manage LOS, develop care plans, and support discharge planning. Requires RN license in VA/NC, BLS within 90 days, ACM/CCM certification within 1 year; a $10,000 sign-on bonus is available for qualified candidates.

Qualifications

• Bachelor's Degree in Nursing is preferred.
• Active RN license in VA, NC, or multi-state compact is required.
• BLS certification required within 90 days of hire.

Responsibilities

• Provide and facilitate comprehensive care management and quality outcomes for patients across the continuum.
• Directly support hospitalized patients identified in need of comprehensive case management.
• Promote effective utilization and monitoring of health services, collaborate with the healthcare team to manage care and transitions.
• Develop and implement a comprehensive care plan addressing physical, mental and psychosocial needs of the inpatient/caregiver.
