Job summary

Sentara Norfolk General Hospital is seeking an Inpatient Case Manager RN for a full-time, 40-hour position onsite at Sentara Norfolk General Hospital in Norfolk, VA. Eligible for a sign-on bonus up to $20,000.

Responsibilities include coordinating comprehensive case management, facilitating patient transitions, and collaborating with the care team to optimize outcomes. RN licensure in VA/NC or a compact license is required. BLS within 90 days; ACM/CCM certification within one year.

Qualifications

• RN licensure in VA/NC or Compact/Multi-state required.
• BLS certification within 90 days of hire.
• Case Management certifications (ACM, CCM, CCCTM, CMAC, CMGT-BC) within one year of eligibility.
• 1 year of nursing experience; 1 year acute care case management preferred.
• Bachelor's Degree in Nursing preferred.

Responsibilities

• Provide and facilitate comprehensive care management across the healthcare continuum, focusing on transitions to appropriate levels of care.
• Directly support hospitalized patients identified as needing comprehensive case management.
• Promote efficient utilization and monitoring of health services; collaborate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs of the patient.
• Function in Hospitals Only setting as part of the care management team.

Sentara Norfolk General Hospital is hiring an Inpatient Case Manager/ Registered Nurse (RN)!

This position is eligible for up to a $20,000 sign-on bonus for qualified candidates.

Status: Full-Time,permanent position (40 hours)

Standard working hours: 8:00 AM – 4:30 PM EST, Monday–Friday, with rotating weekends (Saturday or Sunday). A weekday off is provided when working a weekend day so hours do not exceed 40 per week.

Location: This is an onsite position at Sentara Norfolk General Hospital, 600 Gresham Drive, Norfolk, VA 23507.

Job responsibilities:

• The Inpatient Case Manager RN is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.

• Directly support those in hospital patients identified in need of comprehensive case management.

• Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.

• Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.

• Functions in one of the following practice settings: Hospitals Only

Education:

• Bachelor's Degree in Nursing preferred

Certification/Licensure

• Registered Nurse (RN) in VA or NC or Compact/Multi-state REQUIRED

• BLS within 90 days of hire.
