Job summary

Sentara Norfolk General Hospital is seeking an Inpatient Case Manager RN for a part-time weekend dayshift, onsite at Norfolk General. The role focuses on patient transitions, care coordination, and collaboration with the care team to optimize outcomes.

The successful candidate will have an RN license in VA/NC or multi-state, BLS, and CM certification within a year, with 1 year of nursing and case management experience preferred.

Qualifications

• RN license in VA or NC or multi-state Compact is required.
• BLS certification within 90 days of hire.
• Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) required within one year of eligibility.
• 1 year of nursing experience and 1 year acute care case management preferred.

Responsibilities

• Ensure and facilitate comprehensive care management, services and outcomes across the healthcare continuum.
• Directly support hospitalized patients identified for comprehensive case management.
• Promote appropriate utilization and monitor health services with the care team and patient/caregiver.
• Develop and implement a comprehensive care plan addressing physical, mental and psychosocial needs.
• Operate in a setting focused on hospital-based practice.
