The position requires a nursing license with at least 12 months of experience; certification in ACM/CCM/CCCTM/CMAC/CMGT-BC within a year is required, with BSN preferred.

Qualifications

• 12 months Nursing experience required; critical care experience preferred.
• BLS within 90 days of hire.
• Associates of nursing Required; BSN is preferred.
• Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
• 1-year acute care case management experience preferred.
• Experience with community resources preferred

Responsibilities

• Coordinate comprehensive care management across the hospital care continuum.
• Focus on patient transition to appropriate level of care and clinical appropriateness.
• Support inpatient patients identified for comprehensive case management.
• Promote effective utilization and monitoring of health services and transitions.
• Develop and implement comprehensive care plans addressing physical, mental, and psychosocial needs.
