Job summary

Sentara CarePlex Hospital in Hampton, VA is seeking an Inpatient Case Manager (RN) to support the Emergency Department member population. You will coordinate comprehensive care plans, facilitate transitions, and work with the hospital and health plan teams to ensure high-quality outcomes in a busy acute-care setting.

Requirements include an RN license (multi-state or Virginia), active BLS, and CM certification within 1 year.

Qualifications

• RN license in multi-state or Virginia license.
• BLS certification within 90 days of hire.
• Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) within 1 year of eligibility.
• 1 year of nursing experience (required).
• 1 year acute care experience (preferred).
• 1 year of case management experience (preferred).

Responsibilities

• Support the Emergency Department-health plan member population and coordinate inpatient care.
• Develop and implement comprehensive care plans based on assessments.
• Facilitate safe transitions to appropriate levels of care and collaborate with the care team.
