Sentara CarePlex Hospital in Hampton, VA is seeking an Inpatient Case Manager (RN) to support the Emergency Department member population. You will coordinate comprehensive care plans, facilitate transitions, and work with the hospital and health plan teams to ensure high-quality outcomes in a busy acute-care setting.

Requirements include an RN license (multi-state or Virginia), active BLS, and CM certification within 1 year.

Qualifications

• RN license in multi-state or Virginia license.
• BLS certification within 90 days of hire.
• Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT-BC) within 1 year of eligibility.
• 1 year of nursing experience (required).
• 1 year acute care experience (preferred).
• 1 year of case management experience (preferred).

This position functions as part of our integrated delivery system care coordination model. While operationally aligned to hospital leadership, the role maintains shared accountability to health plan clinical leadership to support high-risk member management, utilization reduction strategies, care transition performance, and enterprise population health objectives.Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in-hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver.
