Job summary

Kaiser Permanente Washington Hospital Center in Washington, DC invites applications for an LISW/LGSW Case Manager in Inpatient II. This 24-hour shift role coordinates care, conducts psychosocial assessments, and connects patients with resources to support independence and quality of life.

The position requires DC licensure and relevant social work experience in healthcare, with a $4,500 sign-on bonus. Part-time schedule; travel up to 10% may apply.

Qualifications

• Minimum one year of clinical social work experience in a health care setting.
• Credentials from the primary work state are required at hire; additional credentials from other states post hire.
• DC licensure within 6 months of hire; Maryland license within 6 months; Virginia license within 6 months.

Responsibilities

• Coordinate and facilitate case management services for defined population.
• Complete psychosocial assessments to inform care plans and optimize outcomes.
• Collaborate with the health care team to develop and monitor plans of care.
• Identify and connect to community resources to meet plan of care.
• Document case management activities per organization standards.
• Analyze patient and program outcomes to identify improvements in quality and cost effectiveness.
• Assist with access to Medicaid/Medicare and other programs where appropriate.
