Sentara Norfolk General Hospital is seeking an Inpatient Case Manager (RN) to coordinate care across the hospital stay and facilitate transition to appropriate levels of care. The role requires an RN with a BSN and active licensure in VA/NC/compact or multi-state, with BLS within 90 days of hire.

A case management certification must be obtained within one year of eligibility. Sign-on bonus up to $20,000 is available.

Qualifications

• Bachelor's Degree in Nursing preferred.
• RN license in VA/NC/Compact required.
• BLS certification within 90 days of hire.
• Case Management certification required within one year of eligibility.
• 1 year of nursing experience; 1 year acute care case management preferred.

Responsibilities

• Provide and facilitate comprehensive care management for patients across the healthcare continuum, focusing on transitions to appropriate level of care.
• Directly support inpatients identified as needing comprehensive case management.
• Promote effective utilization and monitoring of health services; collaborate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement a comprehensive care plan based on assessment and evaluation of physical, mental, and psychosocial needs.
