Sentara Obici Hospital in Suffolk, VA seeks an experienced Inpatient Case Manager (RN) to coordinate comprehensive care for inpatients and ensure appropriate transitions across the continuum. The role emphasizes collaboration with the care team, discharge planning, and optimization of LOS with clinically appropriate services.

The ideal candidate has an RN license, BSN preferred, and CM certifications within a year.

Qualifications

• RN with appropriate degree(s) and Virginia licensure.
• Certification in Case Management required within one year of eligibility.
• At least 1 year of acute care case management preferred.
• BSN preferred; strong communication and interdisciplinary collaboration.

Responsibilities

• Facilitate comprehensive care management and transitions for inpatients.
• Promote appropriate utilization of health services and monitor outcomes.
• Develop and implement care plans addressing physical, mental, and psychosocial needs.
• Collaborate with care team and patient/caregiver to coordinate discharge planning.

Directly support those in-hospital patients identified in need of comprehensive case management.
