Job summary

Sentara Careplex Hospital and Sentara Louise Obici Hospital in Virginia are seeking an Inpatient Case Management Supervisor (RN) to lead the case management team, drive discharge planning, and optimize patient flow. The ideal candidate has 3+ years in case management, prior supervisory experience, and an RN with a Bachelor's degree; CCM/ACM/CCCTM certifications are preferred within 1 year.

Qualifications

• 3+ years case management experience (required).
• Previous supervisory experience (preferred).
• Specialty certification within 1 year of eligibility (ACM, CCM, CCCTM, CMAC or CMGT-BC).
• BLS required within 90 days of hire.

Responsibilities

• Supervise day-to-day workflow and performance of the case management team.
• Ensure effective discharge planning and coordination of care.
• Support length-of-stay management and patient flow initiatives.
• Provide coaching, mentoring, and performance feedback to staff.
• Address employee relations matters in collaboration with leadership and HR.
• Ensure compliance with departmental standards, policies, and regulations.
