Pyramid Healthcare, Inc. is seeking a Case Coordinator to join our inpatient team and support patients’ transitions of care by coordinating aftercare services and maintaining communications with referral sources.

The role focuses on ensuring continuity throughout the treatment process, including discharge planning. Responsibilities include arranging aftercare, scheduling appointments, and collaborating with clinical and operational teams to promote successful recovery outcomes in a structured,

Competenze

• High school diploma or equivalent required.
• Experience in behavioral health, case management, or healthcare coordination preferred.
• Strong organizational and time management skills.
• Effective communication and interpersonal abilities.
• Ability to work in a structured, fast-paced inpatient environment.
• Basic computer and documentation proficiency.

Mansioni

• Coordinate aftercare planning to ensure patients have access to appropriate next levels of care.
• Schedule appointments, meetings, and follow-up services for patients.
• Arrange halfway house interviews and placements as part of discharge planning.
• Communicate with referral sources, families, and external agencies regarding patient progress (as appropriate).
• Obtain and maintain necessary documentation, releases, and records.
• Support patients in navigating resources to promote successful recovery outcomes.
• Maintain accurate records and documentation related to case coordination activities.
• Collaborate with clinical, medical, and operational teams to support patient care.
• Perform other duties as assigned.
