Schedule: Monday – Friday | 8:00 AM – 4:00 PM

Pay Rate: $15 per hour

Case Coordinator – Pyramid Healthcare at Duncansville is committed to providing comprehensive, client‑centered treatment for adults facing Substance Use Disorders. In this role, you will play a vital part in supporting patients’ transitions of care by coordinating aftercare services, maintaining communication with referral sources, and ensuring continuity throughout the treatment process.

Key Responsibilities

• Coordinate aftercare planning to ensure patients have access to appropriate next levels of care.
• Schedule appointments, meetings, and follow‑up services for patients.
• Arrange halfway house interviews and placements as part of discharge planning.
• Communicate with referral sources, families, and external agencies regarding patient progress (as appropriate).
• Obtain and maintain necessary documentation, releases, and records.
• Support patients in navigating resources to promote successful recovery outcomes.
• Maintain accurate records and documentation related to case coordination activities.
• Collaborate with clinical, medical, and operational teams to support patient care.
• Perform other duties as assigned.

Qualifications

• High school diploma or equivalent required.
• Experience in behavioral health, case management, or healthcare coordination preferred.
