Responsibilities

• Directs patient care services for the team, including volunteer and bereavement, integrated with the interdisciplinary plan of care.
• Reviews pending admissions, new admissions, and alive discharges to confirm eligibility and appropriateness.
• Collaborates with the team physician and attending physician on palliative care measures for pain and symptom management.
• Ensures complete, accurate data and documentation for recertification decisions in a timely manner.
• Reviews imminent alive discharges to support effective discharge planning.
• Monitors team services (staffing, meds, supplies, level of care changes) for appropriateness and responsiveness.
• Manages the team schedule and coordinates services 24/7 for all patients on the team.
• Facilitates team meetings to incorporate interdisciplinary input into the plan of care as needs evolve.
• Participates in on-call rotation.
• Interviews, selects, trains, supervises, evaluates, and dismisses team staff with the Patient Care Administrator.
• Ensures staff competence through field visits and in-services aligned with hospice principles and team growth needs.
• Acts as a resource and mentor for staff re clinical issues and service behavior.
• Oversees staff and volunteer schedules, workload distribution, and territory assignments to meet productivity expectations.
• Reviews and approves payroll with accurate time and expenses.
• Attends in-services and workshops; pursues professional growth; mentors new Team Managers.
• Monitors clinical records to ensure accuracy and compliance with policy and regulatory bodies.
• Develops and implements performance improvement activities and participates in quality programs.
• Ensures team completes patient/family satisfaction surveys and applies quality improvements.
• Assists with surveys from regulatory bodies and performs substantive chart reviews to show quality care.
• Participates in Outcomes Management and annual program review; conducts utilization reviews for all patients.
