Job summary

Qualifications

  • BSN required; Masters in Nursing or related field preferred.
  • 5+ years of healthcare experience required.
  • 2+ years of experience in case management preferred.
  • Experience with electronic medical records required.
  • Computer proficiency in Word and Excel required.
  • Bilingual English and Spanish preferred.

Responsibilities

  • Provide care management to high-risk patients to improve chronic illness management.
  • Conduct case-finding, assessments, education, counseling, and other interventions.
  • Assist patients in navigating the NYP service delivery system.
  • Connect patients and their families to community resources.
  • Ensure the proper continuation of quality care.
  • Support the hypertension management initiative.
