Lehigh Valley Health Network is seeking an Inpatient Care Manager - Registered Nurse for a full-time day shift. You will coordinate comprehensive patient care across the continuum and lead discharge planning with the interdisciplinary team.

Qualifications

• Bachelor’s degree in Nursing and valid RN license in PA or NJ, depending on hire.
• 3 years of care coordination, disease management, home health or hospital experience.
• 1 year EMR experience and ability to use health IT systems.

Responsibilities

• Assess, plan, implement and evaluate individualized care plans.
• Coordinate safe and timely discharge planning and post-acute needs.
• Document care in the EMR and collaborate with interdisciplinary team.
• Educate patients and families, support adherence to discharge plans.
• Lead and participate in multidisciplinary rounds and family conferences.
