Qualifications

• Master’s Degree in Social Work is required.
• Ability to interact with diverse populations as part of care coordination.
• Knowledge of computer applications and analytical tools.

Responsibilities

• Conducts comprehensive assessments at patient entry to determine length of stay and discharge needs.
• Coordinates safe and timely discharge planning and post-acute arrangements.
• Monitors patient progress through daily review and rounds; identifies barriers to discharge.
• Advocates for patients and families, addressing clinical, educational, and psychosocial needs.
• Collaborates with multidisciplinary teams and coordinates conferences to progress care.
• Maintains knowledge of Medicare/Medicaid and payer benefits and methodologies.
• Educates patients/families to support self-management and discharge plans.
