Job summary

Qualifications

• Diploma or associate degree in nursing and active RN license.
• At least one year of discharge planning/care coordination experience.
• Bachelor's in social work or related health field; Master's degree preferred.

Responsibilities

• Identify clinical, psychosocial, financial and spiritual needs guiding the plan with the patient and family.
• Complete detailed assessments to determine discharge needs and post-hospital services.
• Facilitate teams to develop safe and efficient discharges and coordinate post-hospital care.
• Communicate clearly with patients and care teams to develop individualized discharge plans.
• Serve as liaison between hospitals and post-hospital care facilities and physicians.
