Job summary

Lehigh Valley Health Network in Bethlehem, PA seeks an Inpatient Care Manager-MSW to coordinate comprehensive care for designated patients across the continuum. You will assess, plan, implement, and evaluate individualized care plans while ensuring safe discharge and resource optimization.

As a clinician, care manager, and educator, you will collaborate with the interdisciplinary team to achieve high-quality outcomes and cost-effective care. Sign-on bonus eligible.

Qualifications

• Master's Degree in Social Work is required.
• Less than 1 year experience in an MSW internship program.
• Ability to interact with persons from diverse backgrounds.
• Ability to set priorities to coordinate care plans efficiently.
• Knowledge of computer applications and analytical tools.
• Proven leadership skills.

Responsibilities

• Conducts assessments at entry to determine LOS, discharge needs, and resources.
• Coordinates safe and timely discharge planning including post-acute needs.
• Monitors patient progress and facilitates rounds to remove discharge barriers.
• Advocates for patients and families, reflecting preferences in care plans.
• Collaborates with multidisciplinary teams to implement care plans.
• Refers patients to community-based services to meet all needs.
• Maintains knowledge of Medicare/Medicaid and payer requirements.
• Educates patients/families to support self-management and adherence.
