Responsibilities

• Perform utilization review and discharge planning for inpatient admissions.
• Apply Interqual, AEP and clinic criteria to determine appropriate care.
• Lead healthcare team to select appropriate care options.
• Collaborate with Community Case Managers on high-risk patients.
• Coordinate with Post Acute Care to ensure timely access to services.
