Job summary

AdventHealth in Celebration, FL is seeking an RN Case Manager to coordinate discharge planning and post-acute care across inpatient, observation, and ED settings. You will lead multidisciplinary rounds, assess holistic discharge needs, and partner with patients, families, and paying sources to ensure timely transitions.

You will educate patients and families on illness impacts, mobilize community resources, and document plans and barriers to support quality outcomes in a dynamic healthcare

Qualifications

• RN with active license and at least 2 years of medical/hospital nursing experience.
• Care management/utilization experience preferred.
• Knowledge of CMS Conditions of Participation for Discharge Planning required or preferred.

Responsibilities

• Participate in multi-disciplinary rounds to review patient status, care needs, and discharge plans.
• Educate patients and families about illness impacts and mobilize community resources.
• Assess readmission reasons and coordinate care transitions to avoid delays.
• Organize care conferences with the multidisciplinary team.
• Document discharge planning evaluations, barriers, and post-acute needs.
• Assess discharge needs holistically across inpatient, observation, and ED settings.
• Develop discharge plans with contingency strategies and post-acute arrangements.
• Coordinate communication with post-acute services to ensure continuity of care.
