Job summary

Advocate Health is seeking a Social Work Case Manager to provide psychosocial assessments, care coordination, and discharge planning for patients and families. The role collaborates with the multidisciplinary team to support patient progression and access to community resources.

Role requires a CSWE-accredited MSW and preferably clinical licensure or ACM-SW certification. This position supports Advocate Health services in Winston-Salem, with full-time hours and 40-hour weeks.

Qualifications

• Master's degree in Social Work from CSWE-accredited program.
• Clinical licensure preferred.
• ACM-SW certification preferred.

Responsibilities

• Identify patients who would benefit from Case Management through initial discharge needs screening.
• Assess information by interviewing patients and families and evaluating needs.
• Complete psychosocial assessments to identify emotional, social, and environmental needs related to diagnosis and treatment.
• Develop and implement psychosocial treatment plans including crisis intervention, individual/family therapy, and grief counseling.
• Initiate internal and external referrals based on patient needs.
• Coordinate patient and family care conferences as needed.
• Document patient interactions in the EMR per departmental standards.
• Advocate for patients' unmet needs and collaborate with the multidisciplinary team to discharge plans.
