Job summary

Qualifications

• Master's degree in Social Work required.
• Two or more years’ experience working with patient population preferred.
• Licensed social work licensure as LMSW/LCSW/LSW may be required by state regulations.
• CPR and de-escalation/restraint certification may be required (training available upon hire).
• First aid may be required based on state or facility.

Responsibilities

• Conduct psychosocial assessment of patient and develop an effective treatment plan.
• Obtain information on patient needs, skills, interests, and limitations for treatment planning.
• Provide crisis intervention and assist families in understanding implications of life situations.
• Participate in multidisciplinary treatment team meetings for discharge planning.
• Identify, assess and coordinate patient cases and anticipate needs related to illness, hospitalization, finances, education and services.
• Facilitate education, process groups, support groups and referrals as requested.
• May conduct intakes and assist in coordinating placements within facility programs.
• Promote recovery, symptom reduction and highest level of social functioning.
• Communicate with and support family members; maintain patient records and documentation.
