Job summary

Perimeter Behavioral Hospital of Arlington is seeking a compassionate Discharge Planner to coordinate discharge planning and ensure continuity of care after hospitalization.

You will collaborate with treatment teams, families, insurers, and community resources to develop effective aftercare plans and arrange post-discharge services.

The role requires a Bachelor's degree in a behavioral health field, at least one year in inpatient settings, BLS and HWC certifications, and flexible scheduling.

Qualifications

• Bachelor's degree in behavioral health related field preferred.
• Minimum of 1 year of inpatient behavioral health experience preferred.
• Knowledge of clinical treatment principles, case management, and discharge planning processes desirable.
• Current Basic Life Support (BLS) certification and De-escalation Training: Handle with Care (HWC) certification required.

Responsibilities

• Participate as an active member of the multidisciplinary treatment team and attend meetings.
• Collaborate with therapists, providers, utilization review staff, and insurance companies to coordinate discharge planning and required documentation.
• Assist with medical record completion and chart auditing activities.
• Assess patient needs and work with the treatment team to ensure appropriate services and resources.
• Promote family involvement in the treatment and discharge planning process.
• Coordinate communication with referral sources, mental health professionals, and community agencies to develop comprehensive aftercare plans.
• Arrange discharge services and resources to support patient success after hospitalization.
• Maintain accurate and timely documentation in accordance with facility standards and regulatory requirements.
