Qualifications

Responsibilities

• Coordinate discharge planning from inpatient to community care.
• Assist with aftercare arrangements, scheduling follow-ups and transportation.
• Initiate follow up calls to external referral sources.
• Track discharge data and audit discharge paperwork for accuracy.
• Ensure discharge transition records are sent to receiving providers.
• Perform other duties as assigned.
