Job summary

University Behavioral Health of Denton is seeking a Discharge Planner to coordinate discharge planning for inpatient clients and ensure post-hospital needs are met with continuity of care.

You will collaborate with the Care Center, social workers, and nursing staff to identify resources, communicate progress, and finalize discharge plans.

Requires a Bachelor's in Social Work or related field; at least 1 year hospital experience; Handle With Care training within 30 days.

Qualifications

• Bachelor's Degree in Social Work or related mental health field preferred.
• Minimum of one year hospital experience strongly preferred.
• Handle With Care training within 30 days of employment and prior to assisting in a restraining procedure.

Responsibilities

• Communicate with referral source within the first 24 hours of admission.
• With assistance from the Care Center, social workers, and nursing identify all outside professionals currently working with the patient so that appropriate discharge plans can be completed and facilitates appropriate patterns of communication to minimize duplication.
• Identifies needs of referrals sources and/or professionals during the period of hospitalization.
• Oversees the tasks of continued communication of treatment progress, discharge plans, and time of discharge to outside interested parties as well as facilitating the transfer of required documents to the outside agents.
