Horizon Health seeks a Clinical Assessment Coordinator (Nursing/RN Registered, LPC, LCSW, or LMSW) to oversee the Behavioral Health Fort Duncan Hospital in Eagle Pass, TX. The inpatient unit has 18 beds for adults and seniors.

This role handles referral development, pre-admission assessments and specialized clinical evaluations to inform treatment planning. The candidate must hold a nursing degree or eligible license (RN), and possess a relevant license (LPC, LCSW, or LMSW).

Qualifications

• Bachelor's degree in nursing is required; Master's degree preferred.
• RN, LPC, LCSW or LMSW license required.
• 3–5 years of intake experience required.
• Bilingual with Spanish is required.

Responsibilities

• Diagnose, implements, coordinates and evaluates clinical admission, referral and/or discharge processes.
• Conduct on- and off-site patient assessments with appropriate test administration, interpretation and recommendations.
• Participate in the development of treatment plans in coordination with the Treatment team.
• Enter required data into HorizonPlus system and generate monthly reports.
• Maintain schedule for filing court paperwork and hearings; coordinate with court personnel.
