Job summary

Horizon Health in Eagle Pass, TX seeks a Clinical Assessment Coordinator (Nursing/RN Registered, LPC, or LMSW) for the inpatient behavioral health unit with 18 beds. The role oversees referral development, pre-admission assessments, and complex treatment planning.

Responsibilities include coordinating admission processes, conducting comprehensive assessments, and collaborating with the treatment team to secure appropriate care.

Qualifications

• Bachelor's degree in nursing required; Master's preferred.
• RN, LPC or LMSW license required.
• 3-5 years intake experience required.
• Bilingual Spanish required.

Responsibilities

• Diagnose, coordinates and evaluate admission, referral and discharge processes.
• Conduct on-site and off-site patient assessments with tests and interpret results.
• Develop treatment plans with the Treatment team and coordinate with physicians.
• Use assessment tools like GDS, Zung, LUMS, and alcohol screens.
• Communicate with referral sources, families, and other facilities.
• Review patient accounts for eligibility and authorization daily.
• Maintain court paperwork schedules and coordinate with court personnel.
• Enter data into HorizonPlus and generate monthly referral/admission reports.
• Perform other duties as assigned.
