Job summary

Fresenius Medical Care in Rock Hill, SC is seeking an RN CAP 2 to provide and coordinate clinically competent care for an assigned patient group within the Clinical Advancement Program. The role supports education, leadership, and quality improvement activities within the kidney care team.

You may delegate tasks to direct patient care staff and collaborate with physicians, nurses, and facility staff. Will travel between facilities as needed and participate in on-call rotations.

Qualifications

• Entry level for RNs with minimum 2 years or more of Nephrology Nursing experience in the last 2 years as an RN.
• Acute dialysis experience is preferred but not required.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.
• Perform ongoing collection and analysis of patient data pre – during – post treatment and adjust treatment plans as indicated.
• Educate patients and families on basic learning needs.
• Direct and provide safe, effective patient care for prescribed dialysis procedures.
• Coordinate communication with dialysis providers and facility personnel to ensure continuity of patient care.
• Recognize patient status changes and report to the health team.
• Serve as a resource and mentor for new employees.
