Job summary

Humana Inc. through OneHome is seeking a licensed Pharmacist to ensure safe and appropriate distribution of medications in home infusion settings. The role involves sterile compounding, order processing, inventory management, and strict adherence to USP 797/800 and ACHC standards.

The Pharmacist will monitor lab values, educate staff and patients, and collaborate with physicians and caregivers to optimize pharmacotherapy and ensure compliance with all regulations.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active pharmacist licensure (Pharm.D., Rph) in state of practice.
• Demonstrates sufficient knowledge of sterile IV admixture and is well versed in USP 797 and 800 guidelines.
• 1-2 years of clinical pharmacy experience in home care, hospital, or other infusion setting.

Responsibilities

• Maintains sterile IV admixtures, aseptic technique, and environmental monitoring per USP 797/800.
• Designs and monitors patient care plans tailored to medical conditions.
• Gathers medical information from caregivers, case managers, physicians, and patients to determine suitability for home care.
• Performs advanced pharmaceutical calculations and monitors pharmacokinetic principles.
• Educates staff and patients on pharmacotherapy and collaborates with healthcare professionals.
