Job summary

Option Care Health, Inc. is seeking an entry-level Clinical Pharmacist to provide safe and effective pharmacy services for home care patients. This role coordinates with Nursing, designs care plans, educates patients and caregivers, and ensures proper dispensing in compliance with policies and regulatory standards.

The position requires a PharmD, active license, and knowledge of IV/sterile compounding or MTM. Prior residency or APPE in a pharmacy setting is a plus.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: (a) at least 1 year in sterile IV admixture, (b) knowledge of non-sterile compounding/immunizations/MTM, (c) ASHP residency, (d) APPE with ACSP, or (e) equivalent experience approved by leadership.

Responsibilities

• Determines suitability of patients for home care and collects patient data.
• Designs, monitors and adjusts patient-specific pharmacy care plans.
• Provides medication-use education to staff, patients and caregivers.
• Ensures safety and accuracy of dispensing, labeling and storage of medications.
• Maintains regulatory compliance and confidentiality.
