Soleo Health is seeking a full-time Infusion Patient Care Coordinator to support its Specialty Infusion Pharmacy in Boise, ID. This role focuses on data entry in the clinical management system, efficient scheduling for the nursing department, and proactive coordination of patient care.

Responsibilities include managing referrals, verifying benefits, coordinating with prescribers, and maintaining excellent patient service throughout the care continuum.

Qualifications

• Prior healthcare scheduling experience required.
• Home infusion experience preferred.
• On-site work with patient interaction in a clinical setting.
• Excellent communication and multitasking abilities.
• Basic computer skills including MS Office.

Responsibilities

• Enter clinical information into the management system to enable billing and scheduling for the nursing department.
• Coordinate scheduling activities for the nursing team and ensure accuracy and efficiency.
• Serve as single point of contact for referral activity with patients, prescribers, and providers.
• Provide exceptional customer service and resolve requests in a timely, accurate manner.
• Notify patients about financial responsibilities, coverage, and payor authorizations.
• Coordinate and plan nursing visits and ensure timely medication dispensing and care coordination.
