Job summary

UC San Diego Health’s Infusion Pharmacy seeks a motivated pharmacist to join our team providing comprehensive pharmaceutical care to patients receiving chemotherapy infusions in ambulatory oncology settings at Moores Cancer Center and Koman Outpatient Pavilion. Responsibilities include regimen review, admixture and dispensing, patient counseling, and preceptorship of residents.

The role supports disease teams and committees to enhance medication safety and oncology care.

Qualifications

• Doctorate of Pharmacy degree.
• California Pharmacist license (RPH).
• Can meet minimum experience via PGY2 or PGY1+1 year or 4 years pharmacist experience.

Responsibilities

• Review and verify chemotherapy regimens for accuracy and appropriateness.
• Oversee preparation and dispensing of chemotherapy medications.
• Provide comprehensive patient counseling on medications to infusion center patients.
• Precept pharmacy residents and students; provide mentorship.
• Participate in Disease Team lead pharmacist, Cancer Center, and Department of Pharmacy committees to improve med safety and care.
• Contribute to development and implementation of oncology-related medication use policies.
• Engage in quality improvement initiatives to enhance safety and effectiveness of oncology care.
