Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or infusion suite under a physician's plan of care.

You will perform treatments, educate patients and families, document care in real time, and collaborate with the clinical team to ensure safe, compliant care. Travel up to 75% is required; active RN license and CPR are required.

Qualifications

• Active, unrestricted RN license in state of practice.
• Minimum 1 year nursing experience.
• Current CPR certification.
• Valid driver's license with access to a vehicle for travel.

Responsibilities

• Coordinate and provide intermediate direct home care per physician plan of care.
• Educate patients and families; modify plan as needed.
• Prepare real-time clinical documentation for billing and compliance.
• Coordinate discharge planning and hand off to the next care team.
