Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an alternate infusion suite, under physician plans of care and supervision.

The role requires an active RN license, 1 year of nursing experience, and CPR certification. Travel up to 100% with strong emphasis on patient safety and documentation; certification and training are ongoing.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.
• Demonstrated competency in patient care standards and infusion skill sets.
• Must have valid driver's license and vehicle for travel.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments.
• Organizes and participates in direct patient care, administers medications, educates patients and families.
• Provides training and mentorship to Infusion Nurse I’s as needed.
• Facilitates effective teaching for patient and family to manage prescribed therapy.
• Communicates with clinical team, physicians, patients and families to ensure continuity of care.
• Coordinates discharge planning and prepares discharge summaries.
• Maintains compliance with agency policies and documentation requirements.
• Performs other related duties as directed by supervisor.
